A decade-long study at Cairns Hospital has revealed a remarkable achievement in the treatment of leptospirosis, a potentially deadly tropical disease. The study, which reviewed 60 patients admitted to the hospital's intensive care unit (ICU) with confirmed leptospirosis between 2015 and 2025, has shattered the grim statistics associated with this bacterial infection.
Leptospirosis, caused by contact with the urine of infected animals or contaminated soil and water, has long been a concern in Far North Queensland, where it is diagnosed more commonly in agricultural workers. Symptoms can range from fever and severe headaches to muscle aches, vomiting, and red or bloodshot eyes. In severe cases, the infection can lead to multi-organ failure and even death.
However, the study's findings are nothing short of extraordinary. All 60 patients in the study were safely discharged from the hospital and returned home, achieving a 100% survival rate for cases requiring ICU care. This is a stark contrast to the fatality rates in countries with fewer resources, which can exceed 50%.
The patients were managed using modern principles of sepsis management, with 80% requiring blood pressure support, 27% needing mechanical ventilation, and 20% requiring dialysis. These findings highlight the strength of Australia's healthcare system in managing even the most critical cases of leptospirosis.
Dr. Cath Tacon, ICU clinical director, emphasized the significance of the study's results. "The fatality rate of severe leptospirosis in countries with fewer resources can exceed 50%, but this is the first ever series to describe a 100% survival rate in cases requiring ICU care," she said. This achievement underscores the importance of early recognition and prompt transfer for specialist care in achieving excellent outcomes.
Dr. Josh Hanson, a co-author of the study, stressed the seriousness of leptospirosis and its potential impact, even in high-income countries. "Leptospirosis is not just a disease of poorer countries," he said. "It can also have devastating effects in high-income countries like Australia, particularly in wet tropical regions where the risk of exposure is greater."
Hanson added, "However, this study shows that local health workers are familiar with leptospirosis and are able to recognize it promptly and deliver optimal care. We hope that the management principles outlined in this research can be used in other parts of the world where leptospirosis is encountered and where deaths are more common."
Dr. Simon Smith, another co-author, emphasized the importance of prevention. "Preventing leptospirosis starts with education and awareness," he said. "Simple measures like avoiding contact with floodwaters and ensuring workers at risk of exposure are wearing protective clothing can significantly reduce the risk of infection."
